This form should only be filled out by an official of the school or college, such as the principal, teacher, guidance officer or admissions officer. It constitutes a licensing agreement between your institution and COLLEGE BOUND Publications, Inc.

This subscription allows anyone associated with your institution to gain immediate access from the school or college computers or from their home computers to collegeboundnews.com. Instead of one hard copy of CB for one counselor, for example, anyone in your institution will be granted access to CB's timely reports on-line. We will immediately send you your collegeboundnews.com access code upon receipt of your check or purchase order.
